Substance Use Evaluation &
Treatment Planning

Assessment and Consultation

Substance Abuse Evaluations (SAE’s) are also known as “drug and alcohol evaluations”, or “DUI assessments”.  These are brief evaluations conducted by a licensed mental health provider with experience in the assessment and treatment of addiction issues.

 

I offer 2 different options with regard to the SAE process. The Basic Evaluation and the “High Touch” Evaluation and Recommendations process.

 

The Basic Evaluation:

Consists of scheduling a 45-to-60-minute appointment. Intake documents and questionnaires must be completed prior to the assessment appointment, otherwise your appointment will be rescheduled. During the assessment I will conduct an evaluation which includes questions that measure your risk level. Following the appointment, I will complete a brief written report that will include a summary of your evaluation and treatment recommendations. Your completed report will be emailed to you within a week. I understand that often these evaluations are time sensitive due to scheduled court hearings. If you require an expedited evaluation, or report, please let me know during the initial scheduling process.

 

The cost for a Substance Abuse Evaluation and report is $600. If we have discussed and been able to extend an Expedited Evaluation, fees for this will depend on weekday and weekend schedules. Additional fee for Expedited Process average about $350. Payment is due at the time of session. Follow-up evaluations and/or reports, which are typically conducted 30 days after the first appointment, are $450 per update. The cost of evaluations are not reimbursable by insurance.

 

High Touch Substance Abuse Evaluation:

Consists of every aspect of the Basic SAE. Additionally, it includes an interview process with up to four people involved in the case for up to 30 minute conversations with each. This may include therapist, psychiatrist, school counselor, parents and/or spouse. Also included is a 30 minute result of findings session with the client as well as any family members the client would like to include. This session is intended to support implementation of recommendations and answer questions. The cost for the High Touch Substance Abuse Evaluation is $1200. If we have discussed and been able to extend an Expedited Evaluation, fees for this will depend on weekday and weekend schedules. Additional fee for Expedited Process average $350. Payment is due at the time of session. Follow-up evaluations and/or reports, which are typically conducted 30 days after the first appointment, are $450 per update. The cost of evaluations are not reimbursable by insurance.

Telehealth Addiction Services

 

There are several potential benefits that come from using virtual services. It allows therapists to connect with people who may otherwise not be able to access services, there is an opportunity for more flexibility in scheduling, and convenience in being able to connect from a space of your choosing.

 

When using online technology there is always the risk of security issues, as well as technical issues (phone not charged, computer or software not working, etc.).

 

In order to protect your confidentiality and to facilitate the security of your information as much as possible, here is a list of recommendations:

 

  • Conduct your session in a private location where you cannot be heard by others. If privacy is an issue due to sharing living space with others, please let me know ahead of time in order to discuss possible options.

  • Use a private phone, tablet, or computer.

  • Do not record any sessions (this violates HIPAA and my terms and the terms of My company and will result in termination of services).

  • Password protect any technology you will be interacting with me on.

  • Always log out or hang up once your session is complete.

  • Keep texts and emails free of sensitive information. Send documents through your portal. (the portal is our HIPPA complaint Electronic Health Records system and safeguard where we communicate sensitive details and keep a client record)

  • Do not share your passwords with anyone. If you would like someone to see a document in your portal log in first then invite them to read/sign. Alternatively, I am able to send forms directly that may be completed and uploaded by you into your portal.
